Title: Pyelonephritis with tubulointerstitial nephritis
Description:
(A): Medullary rays of tubules in normal kidney, stain Azan, human.
(B, C): Pyelonephritis, stain hematoxylin-eosin, human. The nephritis affects the interstitium around the tubules leading to tubulointerstitial nephritis with heavy inflammatory cell infiltrate (lymphocytes, macrophages, giant cells) and eventually replacement by connective tissue causing renal failure. In (B) widened infiltrated collecting tubules with numerous inflammatory cells both in the lumen and in the interstitium. Cortical area in (C) shows lymphoid follicles and massive infiltration of inflammatory cells, including giant cells.
Apart from a variety of glomerular changes periglomerular fibrosis (arrow) is a common finding.
